Fidelis Care at Home has broadened its reach into New York City as well as four new New York counties where it will now provide in-home care to those with chronic illness or disability.
Fidelis, the New York State Catholic Health Plan, will now offer its Fidelis Care at Home Managed Long-Term Care to New York City’s five boroughs as well as Albany, Bensselaer, Schenectady and Montgomery counties in addition to its current coverage area including Orange and Rockland counties. Since inception, the program has grown from enrollment of 244 members in December 2008 to 436 members in March 2012.
The program aligns with initiatives proposed by the state’s Medicaid Redesign Team to bring cost-effective, in-home care to those suffering from chronic illness or disability. To qualify, applicants must be 18 years of age, Medicaid eligible, reside in approved county and be eligible for nursing home level care. Applicants must also require the plan’s services
To qualify for Fidelis Care at Home, applicants must be 18 years of age or older, be Medicaid eligible, reside in an approved county, and be eligible for nursing home level care. In addition, the applicant must require the plan’s services for 120 days or more, and be able to remain in their home without jeopardizing their health or safety.
The broader Fidelis Care network also offers low-cost health insurance plans and has more than 747,000 members statewide.
