Senior Home Care is expanding its Telemonitoring Pilot Program to include three office locations in Florida after launching the service in July 2012 in a bid to prevent avoidable hospital readmissions.
The telemonitoring monitoring program started in Gainesville and after gaining traction with referring physicians and patients has grown to include two more office locations in Florida, in Naples and Fort Myers.
Senior Home Care’s at-home monitoring program targets high-risk patients with chronic conditions by combining clinical visits with telephonic intervention, which has been shown to improve patients’ outcomes, reduce hospital readmissions, and enhance patients’ understanding of their overall condition and progress.
Cardiocom, LLC is Senior Home Care’s telehealth solutions provider. Its Commander FLEX device monitors patients’ vital signs, including pulse, blood pressure, daily weight, and more, and transmits real-time data to Senior Home Care that goes into each patient’s established Electronic Health Record. Clinicians can monitor, review, or follow up on the records as appropriate.

Telemonitoring services are a “valuable tool” for patients recovering from acute illnesses or exacerbations of chronic conditions to self-manage conditions, says Judy Fenton, Senior Home Care’s director of disease management.
“During telemonitoring, our clinicians prepare patients for post-monitoring, so they are able to recognize their key signs and symptoms—such as weight gain and swelling of the feet—that may require early intervention to prevent avoidable hospital re-admissions,” she says.
