Acclaim Health today announced it has implemented CellTrack’s CareManager technology for use among its home care services.
The technology allows its care providers to store digital copies of paper-based client health information to a common server that can be accessed remotely.
Schaumberg, Illinois-based Acclaim Health is a non-profit, charitable organization that provides community and home-based care services. It says the move to a digital platform has helped to boost efficiencies.
“We had close to 60 pieces of paper documentation before implementation and now we can better utilize our time with only 6 easy to use electronic forms and use this time for client care,” said Angela Brewer, CEO at Acclaim Health. “Data is accessible 24 hours a day in real-time, exactly when we need it. Documenting at the point of care is error proof and produces more efficient visits.”
Acclaim equipped each of its 70 visiting nurses with a tablet computer running the Care Manager application, which supports point of care documentation intake from health summary to medical orders and referral documentation. The tracking is all done remotely via tablet.
