Laguna Health Lands $15M, Wants To Become ‘Waze’ Of Hospital-To-Home Transitions

Laguna Health, a post-hospital recovery assurance company, announced a $15 million Series A Tuesday.

The New York-based company plans to use the new funding to further its AI capabilities and help expand its “go-to-market,” co-founder and CEO Yoni Shtein told Home Health Care News in an email.

“The funding enables us to double down on our go-to-market momentum with existing and new partners,” Shtein said. “We are in pilots with additional payers and care providers and look forward to sharing more detail on those in the coming months.”

Laguna’s recovery platform and app use data, digital care tools and behavioral health interventions to reduce negative health outcomes for patients leaving the hospital.

The platform curates a personalized recovery plan for patients to reduce readmission rates and health care costs for patients and insurance partners. The Laguna Health app uses step-by-step instructions that are refined and evolve over the course of treatment to meet the needs of a patient.

By using data analytics and real-time insights, Laguna Health wants to become the “Waze of recovery” in home-based care.

“Going home from the hospital is hard,” Shtein said. “Even in cases where the release to self care at home is cause to celebrate, within a few days the reality of life presents itself. A mother nursing a post-surgery knee injury might have competing responsibilities with career or family. An older person with poor memory might not remember medication instructions. Lack of access to transportation may be the reason a patient is unable to attend follow-up medical appointments.”

Of the estimated 35 million people that are hospitalized in the U.S. every year, two-thirds of those patients are discharged to self care at home. Oftentimes, that process can be confusing and cumbersome for people, especially seniors with complex conditions.

Laguna Health’s app is designed to help ease the burden on the patients and create a more effective and new-age care program.

“Our contextual care model and AI engine are uniquely suited to help patients and care managers navigate the realities of life and life circumstances that so often come up after a hospital stay,” Shtein said. “We integrate medical care plans with social, emotional and cultural factors to support ongoing recovery and to prevent costly readmissions.”

Shtein added that Laguna’s capabilities address and support the “leg of the care journey that desperately needs” support and attention in innovative ways.

“Payers and integrated delivery networks are very interested in the cost savings and scale that a solution like Laguna can provide,” he said.

The funding round was co-led by SemperVirens and HC9 Ventures.
