Post Acute Analytics raises $43 million in funding
Post Acute Analytics (PAA) successfully closed its latest funding round with $43 million, according to Pitchbook. The funding included participation from Humana and another strategic investor, as well as existing investors such as Warburg Pincus, Dorilton Capital and Generator Ventures, the company said in a statement.
The company stated that it will use the new funds to accelerate the growth of Anna, PAA’s clinically intelligent proprietary platform, and to strengthen the company’s partnerships with providers and payers.
“The raise is a strong validation of our strategy, technology and colleagues,” Saqib Akhter, CEO of Post Acute Analytics, said in a statement. “This investment highlights the real-world impact we’re making – empowering care teams with real-time insights, enabling safer patient transitions and generating measurable value across the continuum.”

io Health raises $4 million in funding
io Health, an artificial intelligence (AI) company serving home health providers, has raised $4 million in funding. The company distinguishes itself by providing a transparent AI layer over an organization’s electronic medical record system, ensuring real-time compliance with best practices without requiring a switch to another system or application.
The Pasadena, California-based company was launched in 2024 by Dr. David Bell, former CEO of GrandCare Health Services, with two customers. It has since grown to 15 and aims to optimize point-of-care processes to reduce back-office inefficiencies. The funding will improve the product and expand market reach, according to Bell.
Paradigm acquires Careswitch to focus on payment challenges
Revenue cycle management and claims processing company Paradigm has acquired Careswitch, an AI-powered home care operating system.
Through the acquisition, Paradigm will integrate Careswitch’s advanced AI features and agentic claims processing into its revenue cycle management platform.
“The Careswitch team has the deepest AI expertise in home care,” Paradigm CEO Danny Shapiro said in a statement. “We are thrilled that we can build solutions that not only automate the most complex processes in billing and claims, eliminating errors before they occur, but also harness the power of AI to enhance the human side of care itself, empowering agencies to deliver higher-quality, more personalized support.”
The combined company will accelerate the implementation of AI in claims processing, fraud detection and payment accuracy to deliver faster, more precise, and transparent services for providers and third-party payers, according to a statement.
“Home care is one of the most human, most important and overburdened parts of the health care system,” Careswitch CEO Ilya Vakhutinsky said. “This agentic-first approach is the first of its kind in the industry and we are poised to be at the forefront of home care, taking intelligent automations to an entirely new level with AI.”
VIA Health Partners expands home-based primary care with Equity Health partnership
VIA Health Partners has teamed up with Equity Health, a Denver, North Carolina-based provider of home-based primary care and wellness services.
The partnership enhances Charlotte, North Carolina-based VIA’s capacity to provide personalized, coordinated care, especially for individuals with serious illnesses who prefer to be treated at home, according to the company.
Equity Health delivers medical care where patients live, reducing travel stress, breaking down treatment barriers and maintaining continuity of care, according to the company. By incorporating its team and expertise into VIA Health Partners, patients will benefit from a more connected health experience that includes hospice, palliative, supportive and primary care services.
“Equity Health’s focus on patient-first, in-home care fits hand-in-glove with our mission,” VIA Health Partners President and CEO Peter Brunnick said. “Together, we can offer more services, more expertise and an even stronger foundation of support for the people we serve.”

Homecare Homebase and Element5 announce alliance
Homecare Homebase (HCHB) and Element5 announced a strategic partnership integrating Element5’s agentic AI platform with the HCHB electronic health record, enabling clients to streamline workflows.
Augmenting HCHB’s proprietary platform with Element5’s intelligent agents enables providers to automate complex processes, leading to reduced operational costs, improved workflows and greater transparency, according to the company.

Senior Helpers opens in Grand Rapids
Senior Helpers has opened its newest location in Grand Rapids, Michigan. The location, owned and operated by Joe Gugino, officially began serving the community on Sept. 8.
Gugino is a doctor of physical therapy with more than a decade of experience in home health.
“After years in home health, I’ve seen how much seniors value staying safe and independent at home,” Gugino said in a statement. “Senior Helpers combines that experience with a proven system to give families in West Michigan reliable care and peace of mind.”

PocketRN and Memory Lane Games partner to test dementia care model
PocketRN and Memory Lane Games announced a national strategic partnership to test the Centers for Medicare and Medicaid Services (CMS) Guiding an Improved Dementia Experience Model (GUIDE), which is designed to support people living with dementia and their caregivers.
PocketRN is one of nearly 400 participants developing dementia care programs nationwide, working to enhance care coordination and improve access to services and supports, including respite care, according to a statement.
The partnership will provide access to education and support, such as training programs on best practices for caring for a loved one with dementia.
PocketRN has announced partnerships with several organizations to deliver care through the GUIDE model, including Burkburnett, Texas-based Homewell Care Services and Caring Senior Service in San Antonio.
Palo Alto, California-based PocketRN provides virtual nursing and no-cost dementia care for Medicare members and their families. The Memory Lane Games app consists of games designed to stimulate visual reasoning, memory and speech centers of the brain. The app has more than 100,000 downloads in 100 countries. The company is based out of Douglas, Isle of Man, United Kingdom with a presence in Surprise, Arizona.
